Affiliate link.. inserter?..

I have this report that I would like to use as part of my affiliate program. The thought is that people will get to download it, insert their own affiliate links in the pdf and then share it with whoever.
Now, I am 99% sure that I have seen this done before. But I am struggling to find any service/software that offers this option.
To make it clear:
The PDF would need a few fields (links) that can be edited by anyone, but otherwise be un-editabable.
