Is there any difference or do they mean the same thing?

    Yes, there's a difference.

A disclaimer is essentially saying that you're not responsible if a buyer doesn't end up getting the advertised results from a product you promote.

A disclosure is simply stating that you stand to make a commission from the sale.

None of then products I promote are make money products, so I won't need a disclaimer.

If they make (or even hint at) any sort of promise, you should probably consider a general disclaimer.
