Question about surveys...

Hi folks!

Still kind of new here - this is my 16th post!

So I'm thinking I should do a survey on my site to help me better understand my visitors and potential customers, probably even make some changes that will convert visitors to paying customers.

I was never good at doing surveys though. So, what do I ask them? How do I pose the question, and probably just as important, where on my site do I position the survey box?

I need to come up with two separate surveys, one for the main part of my site and the other, a more serene type of survey that's not too intrusive, on my memorial bookmarks page.

I've used Survey Monkey before so I'm thinking about using that interface, but I still need to know what questions to ask and how to ask them to get the most and best responses.

Any help will be appreciated. The link to my website is in my signature, and the link the memorial bookmarks can be accessed from the Nav.

Also, should I duplicate the survey on my blog as well (also accessible from the main page, right column)?
