I recommend Google Docs, http://docs.google.com/
You can create a document using it, which is like Word. You can add table of contents, and add URL links.
Then you choose "Download as PDF" from the file menu after you finish the document. A PDF file will be downloaded to your computer.
The URL links is clickable in the generated PDF.
It is great for making PDF e-books and reports.
