Missing Conversions?

Has anyone had any experience where they can prove that their conversions are not being recorded?
This week I started promoting two life insurance CPA offers. The conversion was said to happen on submission of the third page of the application. One offer was from Wolfstorm Media and one from Affiliate Wise. They were very similar offers and I had an older list of previous insurance clients to email the offer to. My email instructed clients to email me if they wanted a free quote from multiple companies. I received several requests and subsequently forwarded my affiliate link to the interested clients. Four of which confirmed they had indeed completed the online forms and received the quotes. Strangely enough, two were from the Wolfstorm offer and two were from the Affiliatewise offer.

Imagine my surprise when only one conversion was reported on each network. The clicks were reported but only one conversion.
The affiliate manager at each network gave me the same song and dance even though I had a copy of the quotes complete with customer name and other details.
Has anyone been in a similar situation?
What would you do? :confused:
