Take an ownership of Facebook Pages

Hello Guys, Recently my client would ask me a favor if i could help them transfer the ownership of a facebook pages that created by one of their ex-staff.

The problem arise because the couldn't contact their ex-staff so the facebook pages now become unmanaged and full of ads in their wall.

I googled and don't find the exact case like mine, i'm about to fill the "Copyright infringement" form, but i think it's not appropriate issue as well..

Can anyone give me clue which form should i fill ? What's the requirement ?
it's very hard to know the pages owner.. any idea on how to know the page owner/creator of a facebook pages ?

    I would try to report the page first. I've never done that but I'd expect it would take you to a complaint form of some sort.
