Best Way To Embed Custom Quiz in Wordpress?

Hi,

I want to embed a quiz in WP post. I am not sure if it might even be a quiz. Anyway here is my requirement.

E.G: Personality Development Test

Q1: Do You read Steve Pavlina?
Ans: Yes
Ans2: No

Q2: Are You Confident?

Ans: Yes
Ans 2: No

Q3: How Much Time Do You Need To Re focus?

Ans 1: 1 hour
Ans 2: 1 day
Ans 3: 1 week

Based on the answers, I must be able to output a result like

You know yourself 75%. (Just a random number I choose for this example. I think it can be computed based on score assigned to each answer)

Something like a FB share or twitter tweet might also be helpful.

What is the best Wordpress plugin to achieve this? Or is it better I use a third party and then embed it in my post?

    If you have a quiz script that would function as you described (giving results based on the input - put it in a Page template > create a Page using that template.

    You can add anything else to the template file.

      Sad thing is, I dont have such a script. I am looking for something that will give me the desired functionality on Wordpress.

    I did look at a lot of plugins on WordPress plugin repository, codecanyon, and some third party like quizilla and such. But none exactly matched the requirement.
