I'm starting up a business that will offer high-end patient advocacy services direct to consumers. I like the idea of linking an online payment option to my website, but wonder if prospective clients might be reluctant to make payments starting around $1500 (per year, with a monthly-payment option) via an online merchant account. Any opinions/advice will be much appreciated.

Considering that a lot of people pay all their bills online, not limited to, but including mortgages and car payments, I doubt that you'd experience any drawbacks. But, as with any business, it's always wise to offer alternatives so that your customer can make the choice for themselves.
