Best Way to Make Great Surveys

Hi Warriors,

I'm planning to make a survey or two. I did some web searching and couldn't find anything that suited my purposes, so I'd thought I'd ask you all.

I'm looking for something that can create raw HTML that I can put on my own page. It needs to have the ability to both put out multiple choice as well as forms where people can write stuff.

Does anyone know of anything that could help me do this? Or do I need to do it via HTML from scratch?

I think almost every WYSIWYG editor can do this. As for handling the information, if you want it sent to you by email the WYSIWYG will also do it, if not you need to either learn PHP or pay someone to do it. All the survey tools I know generate a new page or require some knowledge or mySQL.
