Insurance Quotes Plugin

Hi, I'm building an Insurance Quotes website (wordpress) for a client.

I need a plugin which allows me to produce a page to collect certain info before he qives a quote ie name, date of birth etc

I need the plugin to be inserted in a particular page ie the Quotes Page I've set up

Is there such a plugin or do you know anyone that can produce one?
