Urg: URL is coming in Title

Urgent

My url is coming in home page and post title in top of browser.

In home page it is showing site.com
In posts it is showing: title:site.com

How to delete this site.com from title?

Here the title showing at top of browser is: WarriorForum - Internet Marketing Forums

Like this for my site
At top of browser it is showing like: Keyword Review: site.com

I checked in all in seo plugin and other posts Meta Titles whether there is my site.com by mistake, but it is not there.

So how can I delete that site.com in home page and post title.
