Facebook quizzes

Does anyone know how to make Facebook quizzes that end with an email submit button?

Is this quiz a custom made Facebook app? If so, how much can it cost? Anyway for me to create one easily by myself?

Non-Facebook quizzes

For non-Facebook use, how can I made a quiz that also has an email submit at the end of the quiz? Is there a ready made platform I can use?

Thanks for helping me get my head around this.

    There must be thousand tools for this online? But I'm not sure what you're trying to accomplish... do you want to gather emails from ppl who are entering contests?

      That is right. In fact, I am sending them the quiz results to their emails.

      There might be some available online but I have yet to find a ready made template that I can use.

      I'm hoping someone with experience can share how I can make or get one. It shouldn't be difficult but I'm not sure where to start.
