How to maximize commissions from Amazon Shopping Carts

Hello everyone,

This goes back to making money with the Amazon Affiliate Program. I have a couple quick questions for ya... pretty basic.
If "Customer X" clicks on toothbrush that I'm selling on my amazon affiliate site, adds it to cart and decides to purchase everything in their shopping cart including: diapers, baby formula., etc. Do I get commission for just the toothbrush or the toothbrush plus the other products?

If "Customer X" clicks on my toothbrush amazon affiliate add, is taken to amazon, and decides he does not want the toothbrush but buys diapers instead, do i get the commission from the sale.

If "Customer X" goes to my site and clicks on my toothbrush amazon affiliate product icon and adds it to cart via my 90 day cookie, do I get commission on anything and everything that "Customer X" buys for 90 days? Or just on the toothbrush within that 90 days?
I've been working on some basic amazon affiliate marketing and wanted to verify a few things. I thought I knew the answers but my I seem to be making sales and not making the correct commissions.

    If they fill up the shopping cart after they put your tooth brush in: you get all commissions

    If they only add toothbrush to their already full shopping cart after they made an ex-curse to your site: now you only get the pennies for the brush
