Can someone please share How To Make an Image Opt In Box?

Not interested in the boring text opt in code from
Aweber or GetResponse.

I would like to know how to incorporate the code
once I design the image in Photoshop.

How do you make the text fields (name and email)
type ready / editable?

    As far as I know you don't.

    You have to use a combination of the two, use a picture, cut into slices, then arrange them in the html, around your normally editable input fields. You can change the size and shape of the edit fields using css. Make sure to change your background color of the form or table being used for consistency.

        The answer given by ObsidianKnight is the traditional solution to this, and the solution that is the most used. An alternative would be to keep the entire graphic in one piece and overlaying the text portion using css or putting the image in the background of a cell. You can buy pre-made templates that have already done it for you.
