The best way to do this is to use an autoresponder.

You could use a simple form tool like JotForm.com, but that would only be for gathering the info on your site. The real value is in capturing that lead so you can communicate with them over and over again and build a relationship with them.

Generally, the most respected autoresponders are Aweber and GetResponse.

    With Aweber or Getresponse..
Very easy to create a form where you can collect any data you need

I agree with Dave. Aweber will do you just fine and you can even customize the look of your signup form. Doing it yourself with your own HTML website editor may make things look a bit clunky.

    Hi,

It depends on what you want to do with the information.
From that information, it looks like you might be using it to call people up, in which case you'd need it to integrate with a CRM. I haven't tried it, just had a quick look at it, and it appears to do what you'd want it to, you could use WP CRM a plugin to add to your Wordpress installation (http://wordpress.org/extend/plugins/wp-crm/) I have absolutely no affiliation with these guys, never used the plugin, just thought I'd point you in the right direction.

If on the other hand you wanted to automate your communication with these people, you would want to look at an auto responder, such as aWeber. I've used a few, and find aWeber to be the best by far. What this will do is allow you to prepare a bunch of e-mails (called an e-mail series) that get kicked off at intervals set by you once the visitor enters their details. This makes it appear that you speaking to each person directly even though it is completely automated. People are pretty wise to this now, but it is still very effective at creating a big following for you and making sales on auto pilot.

The aWeber software has a built in designer to create the opt-in form that gathers the visitors details for you. Having said that if you tried to capture that much information in most niches, you would reduce your likelihood of subscriptions (people can't be bothered)

Let me know if you need more help or explanation...

Can you have it forward to your CRM without using Wordpress? Because I want to capture the lead / info also from social media pages, various webpages, not just my blog. But yes, auto transfer to CRM would be godsent.

You can used autoresponders like getresponse, sure.

In reality you could use a simple, free solution such as a Google Docs form which you can put on a web page and gather whatever fields you want.

Go to Google Drive and create a "Form" and everything will be pretty self explanatory.

Barbershop, as the others have shared with you, it sounds like to me that you need an autoresponder. Either one of those will be sufficient whether it's GetResponse or Aweber. You can customize those forms to your liking and put whatever fields you like. With Aweber, I think they have a $1 trial for the first 30 days and GetResponse may be free for the first thirty days, but limited on how many subscribers you can have.

Also, Aweber integrates very well with the social sharing sites like Facebook, Twitter, etc. and should be a nice fit for you from that regards. Hope this helps.
