Surveying my Customers - What Questions to Ask?

Today I am giving my product away for free.

At the top of my sales page, I have a huge thing that tells people to follow a link to fill out a survey.

After they've doent his, they will get my product for free.

My main goal here is to use their answers to improve my product and salespage.

But what are the best questions to ask them?

So far I have:

1.) What are their top three fears
2.) What are their top three questions
3.) What are their top three angers (regarding the problem my product resolves)
4.) What would be the ultimate quick-fix to their problem

Anyone have any others I can ask? or should I ask less?

AFAIK, the masters Kern and Dale (used to) advocate asking one question and one question only. Something like:

"Whats the single most important thing you want to know about X?"

I guess the idea is there are lots of benefits to leaving it open to asking for a "thing" rather than fleecing for answers to specific questions. Not the least of which are, people are more likely to fill out a single question survey and ... the results will be as honest and organic as they could possibly be since no real direction is given (e.g. fears, angers, etc.)
