Free Lead Management System

First of all sorry if this is not in the right place.
I'm looking for some platform that I can hire or even some way that I can use to built leads data base but not by creating Form. What I want is to leave that - creat Form - with client and only have to implement some tag or code on Form so we can have leads information. Are you aware of something like that and... for free?
Well even if it's payed please which one is the best in terms of quality vs price.
I guess we need something like this: Free Lead Management Software | Lead Tracking | Convertable but once again that means that we must create Form and we don't want that...
