  To create banners i use Wordpress Header - Blog Header - Web Headers - Custom Designs - Website Banner . They do excellent work for just 11 dollars per banner. Once approved you get them in jpg and payered photoshop file so you can alter them if you like.
  
  Concerning the publishing it depends a bit of the website you publish them on. Some will allow to just upload the banner others would allow only to add the image url. In the latter case you need to have them stored on a webserver.
  
  Depending on the products you advertise these banners might already exist and be hosted somewhere. In this case you only need to use them with your affiliate id.
