Multi-Step Form Services?

Hi guys, do you know of any online subscription services (or anything else you can recommend for that matter) that offers a multi-step/page form?

I want something to work like www.lendingtree.com.

Basically, a few fields on the first page, a few on the next, and then a final step. When they are done, I want all the info sent to me at once.

I am legit going nuts trying to figure this out and can't afford to lose any more hair.

    Forgot to add that I am using Optimize Press 2.0 as my WP theme and I am looking for something that will match the look and feel of the design of the page.

    If you're on WordPress then GravityForms is going to be your best friend. It's probably the best form plugin you could want (and simple to set up and use) and handles multi page forms well. I've used it before to great effect in building multi page input forms for listing sites. WordPress Forms - Gravity Forms Contact Form Builder and Lead Data Management Plugin For WordPress (direct link to their website, not an affiliate link).

    Worth the investment if only to save yourself becoming bald

    Thank you - I will check it out. How is the design aspect of it? Does it take a lot of technical knowledge to make the form "fit the feel" of the page that I am putting it on?

      I created designs quite quickly you can add custom HTML into the actual form itself, if you want to get really custom there is some straightforward CSS, plus they have a great support forum at gravityhelp.com too, it's a very long standing plugin so there are plenty of long term users to help along the way. I don't have a need for it any more but when I used it it really took the headaches away from form building. Building the form is via a drag and drop interface (a little like the OP theme you're using). I'd check with OptimizePress whether there is any known issues with using gravity forms on OP2 (I've just bought OP2) as I know they're still getting the bugs ironed out of the OP2 Theme right now (but that's well documented elsewhere so doesn't warrant going into here...)

    Another obvious solution is to just hire someone on oDesk or Freelancer to code the form for you for $25-30 and be done with it.
