Wpoptins wordpress plugin expert !

I'm a newbie , this is my first attempt to make a squeeze page , I'm currently using the WPoptin 1.3 plugin , here is my problem :
I'm making a squeeze page related to the weight loss niche , when I hit create a new page using the plugin , I don't select any of the pages they give me " privacy policy , terms of services .... " but , They still appear !
I don't have a problem with the privacy policy and terms of service ! I have a problem with the earning disclaimer !!!
my squeeze page is about weight loss , so the earning disclaimer at the end of the page trigger suspicion to me , so it will do the same to my visitors who shall trust me in order to buy from me !!
If anybody here use the same plugin I'm using , please , How can I remove the earning disclaimer from the bottom of the page !
