Can i copy form or questionnaire from other website? - Need advice

hello

i am seo noob and reading lot of seo stuff these days. I have one quick question for SEO gurus.
My website have fresh and original content but i need to create long questionnaire form for clients which is almost same on all niche websites; so my question is if i copy those questionnaire form then would it be considered as plagiarism in googles eye?

    Your best bet would be to contact the webmaster asking if you could post the same questionnaire on your site in return for a "courtesy of" link back to their site. Have you thought of creating your own form asking similar questions?

      I don't know what copy right rules are everywhere in the world, but in the US, its pretty much when you publish it in some way, you hold the copyright.

      If you are going to ask the same questions as the other person in exactly the same way, you might have a problem, but if you are just making your own form from your formbuilder or email responder, you should be OK.

      If you are copying the css and html of the other form, that is copyright protected.

      As far as SEO goes, this isn't going to make any difference whatsoever compared to what you do on the rest of your site.

        Thank you everyone for replies. Actually i am not copying html/css or any coding part. I performed research and picked good relevant questions from multiple websites.
        Questions are simple like "how many pages your website have?, Is it dynamic or static etc.
        my only concern is it should not affect my seo efforts.

          I think you're good with that. You need a questionaire so people can get the services. The folks at Google and Bing and Baidu are plenty smart enough to figure that out.

          If a questionaire is the only page on your site, then you might have a problem, but otherwise, you're good.
