      And I think there should at least be a couple bullet points explaining what exactly the video is going to teach people. Throw in a description.

      Like the already mentioned plus maybe include your call to action in the steps at the top,

      Step1- Enter Your Email
      Step2- Watch Video
      Step3- Take Action

        So the 1,2,3 idea is good.

        But most would agree that the steps need spelling out to people, as in:

        Step1- Enter Your Email
        Step2- Watch Video
        Step3- Take Action ...

        Like cfarmer said.

      Also you might want to test without the name field. You will get a higher optin rate without a name field and on your up sell when people actually buy something from you THEN you can get their name and put them on your buyers list.

      It is great to personalize messages but I have found you don't see much gain from it with freebie seekers. Once you get buying customers THEN personalize things more.
