Polls, Forms - and banning people from joining

Hi Warriors,


This is a multi-topic question that relates to a new PLR site I am planning.
  1. I'd like to add polls and/or a form on my site so that I can get
    feedback from members. What would you recommend as the best
    way to do that?
  2. When people join and then ask for a refund, I want to be able to
    ban them from signing up again. Any idea on how to do that?
In terms of 1, are there any products around that will create a poll/form
that does not require any fancy setting up? I tried to use the form feature
in FrontPage, but I'm completely baffled. In an older version of FP, I read
something about having to set up the bin, and that your host must have
certain capabilities for me to use it.

I'm looking for a plug-'n-play system, if there is one.

My plan is to use DLG to protect my pages - so maybe there's something
in there that can relate to #2, but I haven't found it yet.
