I'm looking for a pretty subscription form template

I've just added a subscription form on one of my niche blogs, and it's just the standard black text autoresponder form. It'll do the job, but iIt's not very pretty!

Can anybody direct me to where I can find some of those really nice subscription box templates, with colours
