Providing personalised email for users

Hopefully someone can help me on this, I would like to offer my paying users an email account - username@mydomain.com
I need a solution that requires as little input on my end as possible, is very user friendly, is easy to setup, has customisable templates re header and footer messages and most importantly can be monetised with 3rd party advertising.
Any suggestions would be much appreciated,
