What questions should I ask?

I am creating a campaign to offer free Google Places listings to local businesses and I am trying to design my opt-in form. Aside from asking for contact information, what questions should I be asking?

I want to use this as an opportunity to not only grow my list but also collect some data to help target my marketing efforts more accurately.

    That's an awesome idea Scott.

    Well, offline businesses think in terms of industry, not niche. So, industry would be good. You can have a box for revenue and employee count. You're going to want address info, website, phone numbers...

    You might also ask what products they sell and who their target market is... That will give you an idea how you can approach them in the future...

    I would first create a poll or questionnaire to gather information like number of employees, annual revenue, how much would you like to grow, so on and so forth.

    Ask for as little information to get them to subscribe as possible, when they opt in they get redirected to the poll or questionnaire where you can ask them all you want.

    That is how I would do it to minimize the intrusiveness that comes with asking a ton of questions right up front.

    Scott,

    If you are referring to building a squeeze page with your Opt-In form on it then go for the PAIN questions

    1. Why are you competitors on Google Places and you aren't?

    2. Do you see a money benefit in being in the 7 most listed services?

    3. XX% of business are found in Google Places, are you there to grab that business?

    Just a few I can think of ..
