Haven't seen a post about an actual proposal for a while, here is a quick outline for a simple 3 option proposal.
The prices, keywords and services obviously can be altered to fit your needs.
For this example, I will use level 1, level 2, and level 3, you could use Gold, Sliver, Bronze, etc. We're assuming it is a House Cleaning Business.

House Cleaning 123 will be responsible for expenses incurred above and beyond these monthly fees. Some examples of these include but are not limited to: Direct Mail Campaigns, postage, domain & hosting costs, printing costs, design costs, PPC ad spending and various supplies.
With any marketing campaign, success will only come from testing and tweaking of new ideas and proper implementation by management and staff. We will increase spending with the ideas that create positive ROI or build better brand awareness, and decrease spending on ideas that don't.
PPC advertising can be implemented right away. Google Places, Yahoo and Bing Local listings can take 2 to 6 weeks to show in the search results, other services can take 2 to 6 months to show improvements and provide results; depending on the level of competition and the search engines.
