Offline lead contract

I was wondering if anyone had or knew of an available contract template to use for my offline lead generation business. I'm currently in the process of closing a deal with a local plumber who will handle all the leads in my area but I would like to get a contract to finalize the deal this week.

I know various WSOs have been released including a pre-written contract to use, so if anyone knows of they can provide me? Or has an available template?
