Contact Form not working in iphone

Hello everyone I have used a build in contact form from Jot Form which I like but it doesn't display correctly on iphones. I really like using jot form, quick easy and easy to up date. Just need to make sure it works correctly on smart phones. The web address is Scott Cherrie Personal Training - My background

    I checked it on android, it's not displayed properly. The bottom of the captcha and the submit button is cut off, covered up by the footer.
