Is there anyone whom might consider telling/showing me how to modify my severside autoresponder optin code?

I need three things...
  1. I need to be able to place the code on any page and NOT have the background box show up. I can change the background color to any color of course but If I wanted to place the code on top of an image background, I would see the box. I don't want that.
  2. I would like to use my own button instead of the browser generated one.
  3. I would like the position of the text fields, input boxes and the button to be more cosmetically pleasing. Code is set for centering but the name and input fields are not centered with the button and text above the input fields.
