PHP/Scripting POST 2fields to Thank You Page

What I've got works perfectly in a single browser (Opera), partly in (IE) and NOT IN (Firefox), (Safari) or (Flock), and I need to get it fully functional in the major browsers. Here is where you can check it out in Opera and see the Name and State fields being captured P1, and displayed P2, in red prior to proceeding through the blue survey, and on to the final Thank You page: Snowboarding Survey 1

I had a guy working with me on this who is a lot more knowledgeable than I am, but it turned out to be a bit past him as well. I really need this functional a few days before the Winter Olympics ends on Feb. 23rd. I'm in need of someone who finds this kind of thing a snap. Here are the details, and you can take a quick look for yourself to see what's not going on:

PRESENT FUNCTIONALITY:
Only the Opera browser works fully displaying fields in RED on P2
While the script works with some browsers it does not work reliably with the more important browers:
1. Internet Explorer 8 Works to display P2 fields; BUT NOT
beyond to display the Survey
2. Google Chrome Does NOT Work to display fields on P2
3. Mozilla Firefox Does NOT Work to display fields on P2
4. Apple Safari Does NOT Work to display fields on P2
5. Flock Does NOT Work to display fields on P2
6. Opera Works all the way to final Thank you

How do I get things to work reliably with the most important browser?

Any clues, alternatives, help, or referrals will be appreciated by this Expression Web code cripple.
