Plugin for Wordpress Comments ?

Hello all

I'm looking for a special plugin for Wordpress: I put dofollow on comments but people who post comments using keywords instead of a name or nickname.

I want a plugin that adds a field to comment form for the associated keyword to the website. A commentary would be published like this:

David from Internet Marketing Forums
(Content of comment)
