Need for Services

Hello,

This is not a wordpress blog or site.

I need to hire a web designer or programmer to accomplish the following task:

1. Make a form interactive i.e.
A. View and print to fill out and snail mail
B. View and fill out online then print and snail mail.
C. View and fill out online then email the form to a specified email address.
D. I have the form made both as a word document and a html webpage file, I will provide the file.

2. Make a second form interactive i.e.
A. View and print to fill out and snail mail
B. View and fill out online then print and snail mail.
C. View and fill out online then email the form to a specified email address.
D. Provide the option online after the form is completed to pay via paypal.
D. I have the form made both as a word document and a html webpage file, I will provide the file.
