Is there anything to help with answering questions and emailing them in?

This is for a non profit organization that I help on a volunteer basis.

The basic situation is that right now events are being organized around the world, and each person who would like to organize one in their area needs to answer about 20 questions and email them in to 3 people.

Ideally, it would be nice to just have a form with boxes to tick for the yes / no choices, and text fields for each of the questions that need a typed answer. The pages on the site are all .php. I do use php includes but have never done anything beyond those in php. I assume that for a form this size it would be a large job, and security is a major concern.

To make the job manageable, probably the best compromise would be to have the questions on a text file, have the applicant type out the answers to the questions, and then maybe paste it all into a text box, click save, and click a button to submit. That button would then send it off to the 3 email addys.

That would only save the person attaching and emailing it 3 times, but it would be a help. Would that be simple to do? Is there a script around that would be appropriate?

The variety of computer skills of the people filling out the form varies widely, from superb to just learning. Simple would be good.

Any suggestions about either of the options, the ideal, the basic, or anything in between?
