A few questions about Forms...

I am trying to set up a Wordpress site for a work project that will have a number of different form types or formats. I would like to have a series of Dropdown Menu's that allow me to do the following:

1). Select any number of options from the drop down menu - but only display the users selections on the Final Form...

2). I would also like to do the same with a dropdown list that allows a user to add numbers (in a list of priorities) and then display only the numbered options on the final form.

3). I would also like to do the same with Radio Buttons.

NOTE: I do not want to show the entire Dropdown menu or Radio Button list on any of the final forms...

Could you possibly recommend either a plugin that I could use to achieve this goal, or; where I can find some code that I can add to a Wordpress site?

In addition to the above I also need to include information like Date, Time, various Paragraph and one line text sections etc...

Any ideas?

P.S - I have had a look into Machform and Gravity Forms and I don't think that they can do exactly what I need to do...

    Some advice... Once you have more then 1 bullet point on your request list... you will not find a plugin.

    To do everything you need, you will have to program it. Try odesk to hire a good cheap programmer

    Yeah, this sounds like a custom solution to me as well. Don't know of any WP Plugin that does all that. For standard forms on WP you could use Formidable Forms or Contact Form 7.

    If you are looking for custom, check out odesk or scriptlance.
