    That's a GetResponse form. Not sure if it's one of their pre-made forms or if it was created from scratch -- I use Aweber so I couldn't tell you. All of the major autoresponder providers have a pretty decent selection of pre-built forms that you can copy/paste directly into your website, if that's what you want to do.

    If you want to learn how to build forms from scratch though, then the following tutorials are a good introduction...

    PHP Tutorial - Forms

    If you DIY, just make sure that you validate each of your form's input fields, so that you don't open up your website to any potentially nasty stuff (cross-site scripting attacks, etc). Google "PHP form validation" for more info on that.

      This might work for you: Formoid - Beautiful CSS Form Generator

      If you're not very familiar with HTML/CSS you could probably find someone to make this for you fairly inexpensively if you have a good idea of what you want.

      I've created a TON of these and they are not too difficult to make.
