Well what is considered perfect optimization is not set in stone. But I like to see how well I rank for my chosen keywords. Then ill see how my site looks and see if I am not using it enough, using it too much, or if the site itself needs to be improved in a way that make it better in search engines eyes.

I consider well optimized to be "Easy to see and understand by google"

None of those things have a thing to do with SEO. You compiled a top 5 list of jack squat.

You'll need to determine a standard or set of rules.

This might give you some ideas:

On Page SEO Techniques To Rank On First Page - 2016 Edition

Scroll down and you'll see an on page SEO checklist.

So, make your list of what you want to see, then go through each thing on the checklist and give yourself a score or edit the pages as needed.

IE: Keyword is in post title at the beginning (check)
Keyword density is x percent (check)

    Right click on a page,
    select 'view source'
hold down the ctrl tab and hit f,
    type keywords,
    hit enter again and again...

It will show you if the keyword is there and where....
    Look to see if it's in h1, title, h2, h3, after alt=.

Look for related keywords on that page.

    That's easy, learn HTML.

An even easier way is to learn HTML and download Screaming Frog (free for 500 internal URLs).

Looking at the plain text version of a webpage is also very useful considering what you see on a live webpage is usually different on the text version which is what bots (ex: Google bot) crawl (text).
