General On page SEO myths/How does google know which keyword i am targeting?

Hello!

I just started learning SEO these past few days and done some of research on On-Page SEO,
however there are a few questions I have about this topic that I hope you can help me out with !

Below this post I will list out some "facts" I read from other articles and please tell me if I am wrong.
I will include in brackets () any question I have about the topic.

1)Content is the most important factor in terms of on-page seo
- when visitors visit my site they are heading here to learn more about my content
- (the main factor is making the visitor stay on my page?)
- image alt text will help me rank for Google images but have little to no effect on rankings
- include out going links to reputable sources like WIKI about your specific topic ( but why? how does it play an effect on my SEO)
- linking your pages together within the content you are writing about ( but why? how does it play an effect on my SEO)
- include your keyword at the top of content rather then bottom

2) Title tag is the most important tag when it comes to On-Page SEO then <h1>,<h2>,so on
-i should always start with h1 tag then h2 tag
-need to contain my keyword of choice from title tag to these h1,h2 tags

3 meta description has no effect on google rankings other then allowing searchers to be drawn to my snippet

Last question!!!

For example i want to focus on keywords " cool white hats"

But my title tag is "NiceClothes | cool white hats and black scarfs"

how does google know which keyword i am targeting if my title is so broad.
Does the content and h1,h2 tag play a role in concentrating my key word to "cool white hats"?
for example:

<h1> you will love these cool white hats </h1>
<p> Damn i love these cool white hats from Valhalla </P>
