    Maybe all the links are pointing towards the homepage only and not on any other internal pages. But its not usual that for any query the homepage will only rank.

      I don't know their past exactly. But recently i saw some websites having great on page and off page but even then google is not indexing them properly.

      for example do a search query of best plastic surgeon in jaipur. In serp, on the first or second page, you will find a website alcstudio.in. I checked out their website they have the specific page for the query with this exact keyword in their url.
