My Top 5 off-page factors (keyword based):
1, Anchor Text from External Links
2, Quantity/Quality of links
3, Link Domain Diversity
4, Distance from Trusted Domain
5, Traffic and CTR data

My Top 5 on-page factors (keyword based):
1, Keyword in title tag
2, Keyword in root domain
3, Keyword in h1 or in bold
4, Keyword use in internal anchor text
5, Keyword in external anchor text

Top 5 non-keyword on-page ranking factors:
1, Unique content
2, Freshness of content
3, Update History (historically how often the content is updated)
4, Links on page pointing to other url's on the domain
5, External links pointing outside of domain

    What would you say are the top 5 on-page factors, and a fifth off-page factor?

      When it comes to the algorithm, there is certainly room for debate.

        Well how will you rank 'content' on the basis of importance? I think it should to be mentioned on your top 5 list.

          I classify content as a "non-keyword" ranking factor

          Top 5 non-keyword ranking factors:
          1, Unique content
          2, Freshness of content
          3, Update History (historically how often the content is updated)
          4, Links on page pointing to other url's on the domain
          5, External links pointing outside of domain

          (I would say the important really drops off after the first two)
