Help! Site with no Google Search Results Preview Image and Doesnt rank for its name

I have a website with a homepage that is indexed, but does not show the Google Search Results Preview Image next to it in the search results (all other pages on the site do have preview images) and Doesn't rank for its name.

It is obviously some sort of penalty. How can I overcome it?

    Do you have WMT then you could find out if there is a message telling you you do have a penalty and maybe some detail about the reason?

    It has been weeks and months. I have only just decided to fix it. I moved a copy of a website that was indexed on a different domain to this domain about 3 months ago, however, only the homepage seems to have suffered. The old domain does not have this content on it anymore. The internal pages on the this (new) domain rank fine.

    Time for a Re-inclusion request?
