I typically focus on 1 primary keyword and a number of secondary keywords naturally follow (usually extensions of the primary keyword).

Focus on one main keyword phrase that will be optimized in your title, and content. Then focus on a couple more keywords in your content. Write to the reader and not to the keywords though. Create multiple articles and feature them on your site and then submit them to article sites. You can target slightly different keywords in each article. Remember to write to the reader, and not to the keywords, but include the keywords.

Make sure your keyword appears in the content of your pages, but don't do that too much, stay natural and use as much similar phrases as you can, I think google cares more about on your content, not only keywords. (e.g if you want to sell something, try to add some extra content in your page that related to the product or the way to use it, how to, where to, etc.)

People will vary with this number, but I like to have a keyword density of about 1-3%. So for every 100 words, my keyword appears 1-3 times. This for your main keyword.

If you have secondary keywords that are closely related to your main one you can also follow the same guidelines. This is Latent Semantic Indexing, LSI.

Readability is the all important factor.

To me when you start getting up to 5% the readability of the content suffers.

Keyword density? It seems like this word will never go away. There are few things we are sure of in the SEO world and one of them is that keyword density is irrelevant. Help spread the word.

You should try to target no more than two or three related key phrases per page of your site. Any more than that, and the purpose of your targeting efforts will be defeated.

Start optimizing for one, and expand from there.

I usually just build inner-pages for new keywords.

You should target maximum 2 keywords on each page. Don't try to target no. of keywords on single page.

Title tags are very essential and basic part of search engine optimization. Title should be meaningful and informative.
Standard length of title is 66 characters. You can use up to 125 characters in title. Google shows only 56 to 63 characters in results. Your main keyword should be on first or second place in title tag. It helps to increase keyword prominence.

You'll get higher search engine rankings and more targeted traffic if you limit your keywords to two or three per page. When you try to target too many keywords (or keyphrases) on one page, it dilutes the keyword density for each of those keywords.

IMO, you can target 5-6 keywords per page. But the keywords chosen should be of high search volume.

In this particular situation webmasters opines differently...

I would not optimize my content for more than 2 primary keywords per page (and a few secondary keywords)

As the number of target keywords increase, quality of content start vanishing and optimization becomes complicated.
