Revamped my site but old title and description is still showing.

I have a site that is indexed but I have revamped it and changed my keywords im targeting so I have new titles and meta tags but when adding social buttons...its still grabbing the old information. How can I change this?

    You can't do anything in this matter. When Search Engines cache your website again (after the changes have made) Search Engines will update their database.

    Sometime it happen because when we launched our new website then Google spider come and evaluate your website after that Google take some action.Don't worry about it..I also face same problem.
