Need Help understanding the biz side of SEO

I find SEO fascinating. I think I know the basics. I was wondering though from the business side of it, how folks do it for others and charge for it.
Isn't everyone basically trying to rank high for the same keywords?
For example, Dentists in Denver, CO. Now an SEO consultant could get all the dentists in Denver, CO as clients but wouldn't they all be competing for the same keywords?
Is this an issue? Where an SEO consultant could pick a niche and wind up with his clients competing for the same keywords?
