Different Organic Ranking in Different Geo Locations

Long story short:

I have a client who pays me for page one rankings. I have a reasonably competitive regional (state) qualified keyword.

I have the keyword on page one. When I change my location to Los Angeles, NY, NJ, MD, and his own state and he is on page one.

However, when he searches, his default location is the town he is in: Anytown, MD for example.

On this search the keyword is on page 2. How can I deal with this issue and get the thing onto page one to match the rest? Obviously I need to update my contract terms, but other than that...
