"Bad links" penalties

There's a lot of "Bro" SEO about the penalties you get for "bad links".
1. Is the penalty from comment spam/low quality sites just for the keywords targeted in the anchor text? So if there's hundreds of links for "black socks", the page won't be penalised for "white socks". Because this is what I've observed.
2. Or does having low quality links as the majority of your backlinks give you a site wide penalty?
3. Or does having over 30%/50% of your inbound links as keywords (any) rather than "click here", "www.sockshop.com" "Sock Shop" "The Sock Shop" (branded/untargeted terms) trigger a site wide penalty.
Seen all of the above claimed, suspect that the truth is much more complicated.
