Recovery from a Google Manual Penalty - Help Needed

My site was given (I presume) a manual penalty by Google back in October 2011 and hasn't recovered since. I tried changing a few things last year and filed a reconsideration request, which was rejected.
I didn't receive a penalty notice via WMT but in response to my reconsideration request I received the Site Violates Googles Quality Guidelines message.
I've since reviewed my website and made some changes. I'd really appreciate if you all could have a look at the site and see if you can spot any issues that I need to address.
I'm not sure if the issue is with my site, backlinks or any other problem.
Any help would be greatly appreciated.
