AD Layout + CPM Ad Chain Help Needed!

To start off with, I have been in the publishing biz for some time now, but while I'm good with content - I am pretty much a newbie when it comes to the intricacies of online advertising.
Here's the deal. I am currently running Adsense, Context Web, IDG Tech & App Nexus on my website. You can find the ad-layout along side their relevant information in the attached image.
I am currently also signed up with but not using; lijit and federated media (separately).
As of now, I generate around 700,000 page views a month. Of which, approx 50% is from US/EU/UK/Aus. While the rest is from Asia (China, India and several other smaller nations).
Now, my questions..
1. How exactly should I chain my CPM based ads? As in, in what priority? Based on the information that I've just shared.
2. Is the current ad-layout correct with maximizing my revenue potential?
