Geo-location Keywords finished?

Lately, those keywords have been taking a hit even more so than late last year. Not all, just those where the business is actually located outside of the city being targeted. They will rank really well for their actual city, but not the nearby metro city.
Additionally, I've noticed that in a fresh browser with no history or anything logged in, sites will rank locally for the root keyword. for example if I typed in Dentist, Google spits out dentists near my actual location. I've been noticing this for a few months(more like 8 months lol), but I wanted to run through more testing and trials before I came here to get "schooled" by you fine people.
I'm wondering if I should start hitting those root keywords more and ignoring the location-specific add-ons when building links. My gut says yeah, but I thought maybe I'd see if anyone else has already jumped on this before I start going full bore.
