How do you set up a webpage with a survey with Wordpress?

I want to set up a webpage that has a survey and, if the person taking the survey gets a certain result, I want them directed to a different page (sales page). How do I set this up?

    Maybe "survey" wasn't the exact word... I want to make it a personality test like the Myers Briggs.

      You need to figure out what possible results can be obtained so that you can group the responses and send the people who score in a particular way to a particular page.

      Basically, it does not matter what their responses are because you are going to control the number of discrete results they can get.

      I am sure this can be done, but it involves custom coding, which honestly I know nothing about, but you might be able to find someone in the Warriors For hire section.
